Construction project managers must notify the Office of Environment, Health & Safety whenever modifying, removing or installing a campus underground storage tank. All required permits must be obtained from the Los Angeles Fire Department.
Purpose of Policy
To establish procedures for construction project managers to obtain approval from the UCLA Office of Environment, Health & Safety (EH&S), Facilities Management and City of Los Angeles Fire Department (LAFD) for the modification, installation or removal of underground storage tanks.
This policy applies to all UCLA construction projects involving the modification, removal or installation of underground storage tanks.
The project manager or other responsible person in charge of the modification, removal or installation is responsible for notifying EH&S and Facilities Management and ensuring that the proper permit application is processed through the LAFD.
Procedure for Processing Permits
Notify EH&S at (310) 794-5369 with the date and the location of the installation. EH&S will give the contractor any further required instructions. The contractor will then contact the LAFD for the proper forms and fee information. Once the contractor has completed the required forms, a copy will be submitted to EH&S and the original, along with any required fees, will be submitted directly to the LAFD. Once the installation or removal is complete, a copy of the final LAFD approval along with any applicable test results must be forwarded to EH&S at the following address:
